Don't wait until the last minute! Your counselor and teachers are asked to write many letters for students so it is advisable to ask for letters well in advance for optimal results.
Tip: Include a self addressed post card (pre-paid post cards are available at the post office) with your request for a letter of recommendation. Since colleges generally ask for letters mailed directly to the college by counselors/teachers and not through a student, ask your counselor/teacher to mail the post card to you once your letter has been mailed. This will serve as a confirmation should the letter get lost in transit. Don't forget to write a Thank You note to your counselor/teacher.
Colleges also want "official transcripts" in a sealed envelope - do not open and break the seal.
